He was placed on scheduled Norco 10/325 mg q6hrs for pain management until his surgery tomorrow afternoon. The pharmacy called the provider to let them know Norco 7.5 mg is on backorder. The attending physician asks pharmacy to convert the pain medication to the equivalent oxymorphone IV dosing. The provider also wants Morphine IV for breakthrough pain. (Dose reduction for cross-tolerance not necessary) (Use 15% of TDD for breakthrough pain)
