PT completed his intake. PT seemed oriented to time, place, and situation and denied any recent police contact. PT presented with full affect and calm mood. PT appearance was disheveled and not appropriately dressed for the weather. PT reported he is currently homeless since the incident occurred. PT declined a referral for project renewal. PT stated ” I’m not going in the shelter, The shelter is bad.” PT reported he was born New York City. PT reported his mother is living in Atlanta and he had also lived there for a a few years. PT reported he is currently employed part-time with food insta cart making deliveries. PT reported he completed high school and attended college for a short time before dropping out. PT reported his mother and friends supports him emotionally. PT reported he has concerns about his health. PT stated he recently applied for both health insurance and SNAP food stamp and is waiting for it to come in the mail. PT stated that when he receives his health insuarnce card he would visit the doctor. PT denied having any concerns about his mental health at this time.
