JJ is a 70 year old African American male with severe osteoarthritis pain. He is currently taking morphine IR 30mg PO q4h. When JJ remembers to take all of his doses of morphine, his pain is well controlled. However, he often forgets to take his medications and ends up in pain crisis. You decide to convert him to the Duragesic patch to improve adherence. What regimens (including dose, route, and frequency) should you prescribe? How should the transition occur?
