Assume that you have met with a small team previously to design a quality improvement project and have developed a plan. Using the knowledge of your previous assignments, outline the following:

the reason for the project (the why)
the impact to your patient population
a discussion of health disparities within this population as it pertains to the project
your plan, including data collection and potential barriers to the project

Your submission is created in a Microsoft PowerPoint presentation or other video presentation tool.
To meet the objective requirements, your presentation is at least six content slides in length, excluding the title and reference slides, and is an audio/visual presentation. It can contain speaker’s notes that you can reference.
