Cardiovascular Case Scenario Henry Faulkner is a 92-year-old male African American admitted with complaints of increasing dyspnea over the past 24 hours, especially with exertion. He has a history of emphysema/COPD, MI, HTN, and HF. Surgical history includes a fracture 10 years ago to his right ankle and right inguinal hernia repair 6 months ago. He has chronic low back pain due to an industrial accident 20 years ago. He has a 70 pack-year history of smoking but quit about 6 years ago following his MI. He is widowed, with no children or family and lives alone with his dog in his own home. He still drives a car and cannot receive “meals on wheels” so he relies on food from the gas station and meals from neighbors. He is anxious, and c/o of dyspnea with minimal exertion. He does not use oxygen at home. His O2 sat on admission was 88% on room air and his respirations were 20 and slightly labored with crackles in the right lower lobe, with bilateral wheezing. The most recent report is he is now on 3L of oxygen per nasal cannula. His O2 sats have been > 90%. Heart rate is irregular at
