Compose a SOAP note: Patient is a 25 year old male who is brought to the office by his care staff from amiable group home.Client consented for staff to be present.As per staff patient has been refusing to take his medications.He also eloped few weeks ago and police had to be called.He was found wandering in the streets at midnight. Client was counselled on importance of medication compliance but he declined to restart taking his medication.
