Problem Statement
Central Adoption Registry (CAR)- This system is used by the adoption office to keep track of children who have been placed for the purpose of adoption and to match them to adoptive parents. This system:
· Registers prospective couples who are willing for adoption
· Matches prospective couples with prospective children based on various acceptance criteria
· Tracks issues of children who are placed on adoption
Post Adoption Registry (PAR)- This system tracks successful adoptions. It maintains historical records of all successful adoptions in the province.
CAR and PAR Solutions are developed on MS Access database and integrated with Lotus Approach database which is very old database and is not supported by:
· Vendors
· Ministry of Central Services IT division (ITD), or
· Ministry of Social Services IT Services (ITS)
MS Access and Approach require double entry across both databases to ensure the two databases are kept in sync with one another.

VIII. Project Plan with Milestones (with Planned Start Date and Planned End Date), and
IX. RACI Matrix.
