Summary (nap.edu) & Crossing the Quality Chasm: A New Health System for the 21st Century Summary prompted the adoption of Electronic Health Records (EHR) to overcome medical errors reported in these articles. However, EHR- related medical errors have also been reported (Electronic Health Record–Related Events in Medical Malpractice… : Journal of Patient Safety (lww.com); EHR-related medication errors in two ICUs – PubMed (nih.gov).
